A Special Word to Surgical Residents and Fellows The operative dictation, or “op note” as it is commonly called, is one of the most important pieces of the medical record that a surgeon creates. In an era when we increasingly rely upon electronic “templates” and check lists, it is especially important to accurately record what actually happened in the oper- ating room. This includes the indications, the findings, the steps in the proce- dure, who participated, and the sutures and devices that were used.
